Basic Eligibility
Any Bangladeshi Nationals are Eligible to Apply for Thailand Medical VISA (having a valid MRP / E-Passport with at least 6 months validity) subject to provide some required Documents.
Thailand Visa Application Form
Visa application form must be filled and signed by the applicant.
Passport
a. Minimum 6 month’s validity required
b. At least 2 Blank pages required
c. Old passport required (if any)
Proof of Occupation
a. Visa request letter from the applicant and a No Objection Certificate (NOC) from Employer.
b. Office ID card / Visiting Card
c. BMDC certificate or letter from hospital if the applicant is a Doctor,
d. A Bar Council Certificate or letter from a law firm if the applicant is a lawyer
e. A recommendation letter from the school / university or student card /evidence of school / university enrolment) if the applicant is a student.
f. Salary accounts Bank Statement (preferably) / Pay Slip / Salary Certificate are required for service holders.
g. Trade license (original notarized English translation and photocopy of the original trade license required) for proprietorship business, Memorandum of Article for limited company if the applicant’s name is not mentioned in the trade license.
Photo Specification
2 copy most recent Glossy paper photo without border (size 3.5 cm x 4.5 cm & white background)
An Active Phone Number
An Active Phone Number of applicants is required.
Round Trip Air Ticket
Applicants must need to confirm or book of a round trip air ticket.
Other information
a. The first applicant can sponsor father, mother, children, father-in-law and mother-in-law. Father, mother and children can sponsor the above-mentioned family members.
b. For documents in other foreign languages (including Bangla) must be translated into English and shall be certified by a credible translator and a notary public
c. Copy of Marriage Certificate (if in Bengali, notarized English translation and photocopy of the original is required) must submitted if spouse name does not appear in the passport
Proof of Medical Needs
a. Copy of a recent certificate of the referral or a letter (preferably original) from a local doctor/hospital (in an official pad with contact number, sign & date), where the patient is being treated which states the patient’s condition; (for first-time traveler)
b. In case the patient receives treatment in a foreign hospital, proof of previous medical treatment in a foreign hospital is required; (for first-time traveler)
b. Recent case summary of medical reports of the patient issued by the hospital; (for first-time traveler)
c. Copy of an appointment letter from a hospital in Thailand mentioning appointment/admission date & accompanying person name(s)
Sponsors Financial Documents
a. Original Bank Solvency certificate and Original Bank statement (At Least for 6 Months with minimum balance of 100,000 for single & Tk. 150,000 with family) are needed to show.
b. If the applicant is a housemaid/attendant, a sponsorship letter & personal bank statement with a solvency certificate are required.
Additional Documents for Medical Evacuation by Air-Ambulance
a. Recent case summary of the patient’s medical reports issued by the local hospital.
b. Copy of recent certificate of acceptance for treatment/invitation letter from the receiving hospital in Thailand confirming the patient’s condition and the necessity of transfer to Thailand. The letter should mention the accompanying person’s name(s) & relation, who will travel with the patient by Air Ambulance.
c. Booking confirmation letter (such as Med letter & Action Plan) of an Air Ambulance.
Visa Fees & Service Charges BDT 5,500
7 to 10 Working Days but it might be more delayed if any unavoidable problems are happened.
